A 32-year old male complained of an irritative cough, chest pain and fever. Laboratory examination showed leucocytosis and accelerated ESR. On chest x-ray films, infiltration from the hilum to the left middle lung field was noted. Hodgkin's disease was suspected from the appearance of the x-rays and a cervical lymphnode biopsy. The mediastinal mass and the left upper lobe were resected en bloc. Histologically, both the mediastinal mass and the lung lesions were Hodgkin's disease (nodular sclerosing type). It is rare for Hodgkin's disease to be present as a mediastinal mass and pulmonary involvement at the beginning of the disease. COPP therapy was started nine days after the operation. The swollen cervical lymph nodes disappeared at the end of 3 courses, and temperature, ESR and leucocyte counts became normal. Complete remission has continued for seven months after surgery during which time five courses of COPP therapy have been administrated.
