Resectional management of thyroid carcinoma infiltrating the trachea

Fifty-eight patients (47 with papillary, 6 with follicular, 2 with medullary, and 3 with undifferentiated carcinoma) were treated surgically for thyroid carcinoma infiltrating the trachea. Thirty-eight had had prior thyroidectomy. Tracheal resection lengths were 1 ring to eleven rings (average 6 rings). After circumferential resection of the involved segment of the trachea, 39 were reconstructed by laryngotracheal anastomosis and 19 by end-to-end tracheal anastomosis. For the 58 patients, the 10-year survival rate was 57.5%. Resection and reconstruction of the trachea invaded by thyroid carcinoma provides not only prevention of suffocation due to airway obstruction but also an opportunity for cure.
