Four cases of Horner's syndrome after extirpation of a mediastinal tumor

Horner's syndrome, as a complication after surgery was recognized in four of 28 patients (14.3%) with mediastinal neurogenic tumors, three schwannomas and one ganglioneuroma, in the right upper posterior mediastinum. The tumors were removed through a posterolateral thoracotomy. Electrocauterization was used in only one patient.<BR>Horner's syndrome appeared 2, 7 and 12 days after the operation and has persisted in three patients. Examination of the pupils with the use of atropine, epinephrine and cocaine indicated injuries in the preganglionic neurons of the cervical sympathetic nerves. It is therefore recommended that dissection of the upper posterior mediastinum and removal of tumors with the use of electrocauterization should not be done. Other methods, such as microsurgery or the partial resection of tumors, are necessary for the prevention of the Horner's syndrome.
