Intraperitoneal chemohyperthermia (IPCH) with mitomycin C (MMC) was used to treat 7 patients with very far advanced gastrointestinal cancers with peritoneal seedings. Resection of the primary tumor have been possible in 2 cases. After temporary closure of the abdominal wall, a 90 to 120 minutes IPCH was performed under general anaesthesia and 32°C hypothermia, through 3 intraperitoneal drainages realising a closed circuit, using 10 mg/1 of MMC in 6 1 of peritoneal dyalisate warmed at the inflow temperature of 46 to 49°C. We observed no mortality neither morbidity. They were only poor and temporary biological side effects and for 6 patients, no malignant cells could be found in ascites after IPCH. In 6 patients, we observed an improvement of Karnofsky indice for 3 to 7 months after IPCH. These results show that IPCH with MMC is a safe and reliable treatment for peritoneal seedings in far advanced gastrointestinal cancers and led us to go on with this new therapeutic device.
