Hyperthermia of Malignant Brain Tumor with Implant Heating System. Histopathological Study.:Histopathological Study

Histopathological sections of 11 brain tumors treated with interstitial hyperthermia of Implant Heating System (IHS) were studied, in which 7 malignant gliomas, 3 metastatic brain tumtrs and a malignant meningioma were involved. Materials were obtained either from operative resection or autopsy following the hyperthermia with IHS. All the cases received repeated hyperthermia, combined with either radiotherapy or chemotherapy. Measurements of tissue temperature disclosed about 42°C and 38°C in the points of 5 and 10mm apart from the implant respectively. All the tumors showed an apparent necrosis around the implant in the areas of 20 × 10mm by one, 20 × 30mm by two and 20 × 40mm by three implants, which were placed in parallel fashion. In a case of metastatic brain tumor, the tumor completely disappeared and was replaced by a cavity with remaining implant. Microscopically the necrosis was well-circumscribed from surrounding brain tumor tissue and generally homogeneous in pattern. In fact no tumor cells nor inflammatory cells were seen inside the necrosis, but only the ghosts of basic structures were seen, indicating coagulation necrosis. In some cases, this necrotic foci were consistent even after several months following the last hyperthermic procedure, suggesting that the necrosis were not readily absorbed nor invaded by the tumor.<BR>In conclusion, the repeated hyperthermia with IHS can create apparent necrosis of the brain tumor around the implant, which is definitely related to the thermal effects of IHS.
