Plasma thromboxane B<SUB>2</SUB> levels in patients with acute cerebrovas cular disorders

In accordance with the release reaction of platelets, thromboxane A<SUB>2</SUB> (TxA<SUB>2</SUB>) is synthesized from arachidonic acid released from platelet membrane. TxA<SUB>2</SUB> plays an important role not only in physiological hemostasis but also in thrombus formation and progression of atherosclerosis as it has potent, vasoconstrictive and platelet aggregating effects.<BR>To clarify the relation between cerebrovascular accidents and thromboxane A<SUB>2</SUB>, we examined the fluctuations in serum thromboxane B<SUB>2</SUB> (stabilized metabolites of thromboxane A<SUB>2</SUB>) levels in 20 patients with cerebral thrombosis (13), cerebral hemorrhage (6) and subarchnoidal hemorrhage (1). Following the attack, blood samples were withdrawn daily for 5, and 7, 10 and 14 days. In cases of cerebral thrombosis, serum thromboxane B<SUB>2</SUB> levels were statistically high at 346+40 pg/ml (mean±SE) (p<0.01), as compared to normal control values of 217±9 pg/ml and these levels remained high throughout the observation period (p<0.01). One patient with cerebral thrombosis, admitted in deep coma, had a high level of serum thromboxane B<SUB>2</SUB>. With improvement in the level of consciousness, the thromboxane B<SUB>2</SUB> levels decreased. But again fell into deep coma and at the time of death the thromboxane B<SUB>2</SUB> level was high. In 6 cases of cerebral hemorrhage, serum thromboxane B<SUB>2</SUB> showed normal levels of 198±9 pg/ml on admission and remained at the same level until the 7th day. These values increased on the 10th and 14th days with a statistically significance (p<0.01, p<0.05). However, two patients with an extensive cerebral hemorrhage died whose level of serum thromboxane B<SUB>2</SUB> on admission was high and continued to increase until death.<BR>Thus, the assessment of levels of thromboxane B<SUB>2</SUB> in patients with an acute cerebrovascular disorder should be clinically useful to determine the severity and prognosis of the disease.
