Effect of epidural anesthesia on blood glucose and insulin level in the patients with liver dysfunction during upper abdominal surgery.

It was said that patients with liver dysfunction often had glucose intolerant tendency. So, we investigated blood concentrations of glucose and plasma insulin levels during anesthesia in the patients with liver dysfunction or without it.Thirty of patients had liver dysfunction (dysfunction group) and the other had normal range (normal group). Blood concentrations of glucose and plasma insulin levels were measured simultaneously in sixty patients under elective upper abdominal surgery.After glucose loading, insulin secretion in dysfunction group was delayed compared with normal group, and the blood concentration of glucose in dysfunction group higher than that in normal group during enflurane anesthesia. However in epidural anesthesia, insulin secretion of both groups were within normal range and cotinuous hyperglycemia was not observed after glucose loading.These findings suggested that epidural anesthesia does not effect on the glucose metabolism and insulin secretion in the patients with liver dysfunction.
