This report is regarding a case which depicts the functional recovery and remodeling of the shoulder after the joint destruction when delayed post operative infection occured. The case patient was a 27-year-old woman who suffered from the recurrent dislocation of the shoulder. She had had a slight pain and poor range of motion since the surgery, and her inflammation reaction was slightly positive, her joint turned in osteolytic change in image inspections. Her JOA score was 57 points. We diagnosed the delayed mild septic arthritis, and operated on her for arthroscopic debridement and open drainage with the penrose drain. We found synovial inflammation in arthroscopic findings and removed the anchors and sutures, but didn't make any repairs. The follow up period was 24 months and she had had no recurrence of inflammation or instability. The final JOA was 93 points. Her humeral head and glenoid had some remodeling, and the erosion points were improved.<BR>The skeleton is a metabolically active organ that undergoes continuous remodeling throughout life. It helps to repair micro-damages in the bone matrix. Bone remodeling serves to adjust bone architecture to meet changing mechanical needs. Bone mass is increased according to local dynamic load for maintaining bearing properties. We succeeded in the complete removal of the bacteria, achieving functional recovery and good remodeling. Controlling infection, appropriate motion stimuli and young age brought about conspicuous improvement both clinically and radiographically.
