We reported 3 cases with rapidly destructive arthropathy of the shoulder, which is a rare condition, causing marked destruction of a joint within months after the onset of symptoms.<BR>Case 1. A 77-year-old female. Radiographs showed that destruction of the shoulder was progressed for only 4 months. We diagnosed rapidly destructive arthritis (RDA) of the shoulder and performed hemiarthroplasty.<BR>Case 2. A 74-year-old female. She had shoulder pain from 7 years ago. She visited a neaby hospital and was treated conservatively with intra-articular injection. When she visited our hospital, she complained of great pain. Radiography and computed tomography showed a break on the left humeral head and bone defect of the glenoid. Radiographs from the previous hospital, showed that destruction of the shoulder had progressed for only 5 months. We therefore diagnosed RDA of the shoulder, and performed total shoulder arthroplasty (TSA) because the rotator cuff was kept.<BR>Case 3. A 75-year-old female. Radiography showed that destruction of the shoulder had progressed for only a month. We diagnosed RDA of the shoulder the same as cases 1,2 and performed TSA. Two weeks after operation, her shoulder was swollen without any pain. Radiography showed subluxation of humeral head and loosening of the glenoid component. MRI of cervical spine showed syringomyelia, so we performed revision surgery.<BR>In case 3, the clinical course and radiographic findings were similar to typical RDA of the shoulder. Therefore, MRI of the cervical spine should be evaluated in rapidly destructive condition of the shoulder joint.
