Acromioclavicular (A-C) joint injury is one of the common traumatic conditions affecting the shoulder girdle. The primary static stabilizers of the A-C joint are the A-C ligaments, and coracoclavicular (C-C) ligaments. The A-C ligaments mainly control motion in the anterior-posterior plane, and the C-C ligaments are the primary restraint to inferior translation of the scapulohumeral complex, or superior migration of the clavicle. Anatomic A-C and C-C ligament reconstructions are surgical procedures to address A-C joint instability. We report a new method for A-C joint instability using artificial ligaments. C-C ligament is first reconstructed using two artificial ligaments. One ligament is passed beneath the coracoid process, looped and tied to the top surface of the coracoid process. The other one is passed through the loop and tied to the upper surface of the clavicle with the acromioclavicular joint repositioned. The acromioclavicular ligaments are reconstructed using artificial ligaments passed through bone tunnels of the acromion and clavicle. The patient demonstrates improvement in pain levels and function. We concluded that this method using artificial ligaments is very useful for A-C joint instability.
