<B>Background:</B> The purpose of this study was to assess the rotator cuff function of throwing shoulder by Cine-MRI.<BR><B>Methods:</B> Thirteen shoulders of thirteen throwing shoulder patients (average age 31.0 years old) were evaluated, and compared with forty shoulders of twenty adults (average age 26.9 years old) without pain in the shoulder. All scans were done on GE Signa MRI 1.5T scanner. We used FIESTA images of MRI to assess rotator cuff movement. An axial scout scan at the middle of the smaller tuberosity of humerus was done. Subjects reciprocated their shoulder from maximum internal rotation to maximum external rotation in 20 seconds in drooping position. We assessed congruity of the glenohumeral joint in rotational motion and rotation angle.<BR><B>Results:</B> In one of thirteen patients, humeral head moved up and down during rotation. We assessed this patient's motion as the loss of congruity. Others could keep congruity during the whole motion. The mean internal rotation angles of patients were smaller than normal group but not significantly. Mean external rotation angles and total range of rotation of patients were significantly restricted compared with the normal group. Three patients had limitation in internal rotation, and three patients had limitation of external rotation. One case, which had Cine-MRI taken before and after rehabilitation, confirmed the improvement of range of motion from -10 degrees to 47 degrees.<BR><B>Conclusion:</B> Cine-MRI could visualize the limitation of rotational motion and poor congruity of the glenohumeral joint of throwing shoulder patients vividly, thus this method is useful to assess the function of throwing shoulder.
