<B>Background:</B> The purpose of the present study was to investigate the anterior capsule elasticity of the shoulder joint in adolescent baseball players using realtime tissue elastography by shear wave.<BR><B>Methods:</B> 42 shoulders of 21 adolescent baseball players (all males; mean age, 14.1 years, range 9 to 21 years) suffering from throwing disorders were evaluated. All sonograms were obtained by an orthopaedic surgeon using Aixplorer (SuperSonic Imagine, S.A.). The patients were placed in the supine position with the arm abducted to 120° and externally rotated to 90°. Images were obtained from the axilla along the long axis of the humerus. We measured elasticity of the anteroinferior capsule of the shoulder joint at the mid-substance and the insertion of the proximal humerus.<BR><B>Results:</B> The average elasticity of the mid-substance of the capsule was 35.7kPa on the throwing side and 35.4kPa on the non-throwing side, and the insertion to the humerus was 45.3kPa on the throwing side and 44.3kPa on the non-throwing side. Mild positive correlation was found between elasticity of the capsule at the insertion to the humerus of throwing shoulder and the patients' age (correlation coefficient: 0.41). There were no significant correlations between other parts and age.<BR><B>Conclusion:</B> In the throwing shoulder, the value of the elasticity of the capsule at the insertion to the humerus has increased with the years. Although it is not apparent whether this is a pathologic change or an adjustment for repetitive throwing, further investigation is needed.
