<B>Background:</B> The purpose of this study was to investigate the clinical results of 5 cases after long head of biceps (LHB) graft by using Yamane's procedure for irreparable rotator cuff tears.<BR><B>Methods:</B> We investigated 5 consecutive patients (mean age: 75 years old), who had massive rotator cuff tears. The average postoperative follow-up was 18 months (range 12 to 24 months). The evaluation criteria were pre- and post-operative Japanese Orthopaedic Association scoring system (JOA score), humeral head elevation by Ohizumi classification, and postoperative integrity of repaired rotator cuff using MRI with Sugaya classification.<BR><B>Results:</B> The mean preoperative JOA score, which was 56 points, improved to 84.3 points at the last follow-up. Humeral head elevation progressed in one. The Sugaya classification were type 2 in 2, type 3 in 2 and type 4 in one. Although one case presented re-tear of tendon and humeral head elevation, the JOA score of all cases improved.<BR><B>Conclusion:</B> For an irreparable massive rotator cuff tear, this procedure is one of the useful choices.
