It is known that elastofibroma is an uncommon benign tumor and it occurs at the inferior pole of the scapula. We report the clinical findings of 5 patients. We performed the excision of 8 elastofibromas in 5 patients. The average age at excision was 62 years (53 to 71). All cases were females. All patients had bilateral lesions. The average follow-up period was 3 years 4 months (6 months to 6 years 2months). All patients complained of neck stiffness and were snapped at horizontal adduction of shoulder. CT showed an ill-defined mass between scapula and rib. MRI showed a crescent mass. The mass showed similar or hypointense signal to skeletal muscle on both T1 and T2 sequences. The mass contained linear hyperintense signal. Surgery was performed in lateral position. We retracted latissimus dorsi and found an ill-defined mass to intercostal muscle and fascia and resected it. Histopathology revealed elastofibroma, stained black with Elastica staining. After surgery, neck stiffness and snapping disappeared. All patients are satisfied. No recurrence has been observed. Elastofibroma is present in the scapular thoracic joint, which has caused the irregular motion of the scapula. By the horizontal adduction of shoulder, scapula is abducted and elevated. Then snapping occurred by extrusion of the tumor from the lower surface of scapula. The irregular motion of the scapula may be a main cause of the symptom.
