Recently, erosive changes of the acromion under surface after arthroscopic rotator cuff repair (ARCR) have been reported. We experienced a case of the bone cystic change in the acromion after rotator cuff repair.<BR>A 47 year old male had right shoulder pain due to exercises with dumbbells and playing table tennis. He had ARCR by the transosseous fixation for a medium size tear of the supraspinatus tendon. All suture strings were No.2 Ethibond. At post-operative 6 months, the condition of his right shoulder joint was good. JOA score was 93.5 points. The condition of the repaired rotator cuff was type 2 according to the Sugaya's classification on the MRI findings. On the other hand, a small bone cystic change in the acromion was detected on the findings of MRI and CT. At post-operative 8 months, he got right shoulder pain without any trauma. The cystic change in the acromion enlarged on the findings of MRI and CT. At post-operative 1 year, he had a 2nd arthroscopic surgery for the curettage in the bone cyst and the removal of the knots. A suture knot on the greater tuberosity was exposed originally. In the histological examination, the contents from the acromion were granuloma-like lesion including wear fragments of Ethibond strings and multinucleate giant cells and macrophages. His right shoulder pain improved after the 2nd surgery. <BR>The wear fragments of Ethibond might cause the osteolysis in the acromion as the bone cystic change after ARCR.
