<B>Background:</B> There are few reports about time-dependent change of postoperative results in arthroscopic rotator cuff repair (ARCR) with preoperative limited range of motion (ROM). The purpose of this study was to evaluate relationship between clinical results of ARCR and preoperative ROM under general anesthesia.<BR><B>Methods:</B> 38 patients who underwent ARCR and who were followed up for more than 2 years were included in this study. ROM under general anesthesia was measured preoperatively, and patients with below 120 degrees of flexion (flex) or 30 degrees of external rotation (ER) were defined as group A. The other patients were defined as group B. Manipulation was performed under general anesthesia in group A. In cases where manipulation was difficult arthroscopic capsular release was performed. Preoperative and time-dependent change of postoperative active flex, ER and JOA score of these 2 groups were compared with each other.<BR><B>Results:</B> The number of patients in group A and B were 16 and 22 respectively. The mean active flex in group A and B was significantly improved at 3 months and 6 months. Significant difference between the two groups was not observed over one year. Active ER was significantly improved in group A and B at 6 months and one year after surgery. Significant differences between the two groups were still observed at one year after surgery. JOA score was significantly improved at 6 months after surgery in both groups.<BR><B>Conclusion:</B> Recovery period of flexion was similar in both groups, but external rotation recovery was prolonged in group A.
