Clinical study of double filtration plasmapheresis on rheumatoid arthritis.-without plasma protein substitues

Double filtration plasmapheresis (DFPP) was performed a total of 166 sessions on 13 patients with rheumatoid arthritis. After 6 months of DFPP, the clinical improvements of morning stiffness (from 132.3 to 43.8min., P<0.05), grip strength (from 75.8 to 105.3mmHg, P<0.01), 25m walking time (from 38.1 to 28.3 seconds, P<0.01), and joint score (from 91.0 to -30.4 point, P<0.01) were observed. According to the criteria of clinical improvements of rheumatoid arthritis described by Yamagata, 5 patients (38.4%) were good, 5 patients (38.4%) moderate, and 3 patients (23.0%) poor. Anemia developed in 5 patients (38.4%) and peptic ulcer in 2 patients (15.3%). Reducticn in renal function was revealed in 2 patients (15.3%). Hepatitis was not recognized. Four methods of DFPP (One way, Recycle, Cryofiltration, and Cryorecycle) were employed for this treatment. The removal rate of globulin was 33.5%, 42.9%, 30.1%, and 36.5%, and that of albumin was 19.4%, 22.5%, 20.2%, and 24.2%, respectively. These methodes could perform without plasma protein substitute for more than six months.We conclude DFPP is an effective therapy for rheumatoid arthritis and can spare plasma protein substitutes.
