STUDY OF PARAMETERS OF ARTI FICIAL PANCREAS WHICH REGULATE BLOOD GLUCOSE DURING NUTRITIONAL TREATMENT

There are some problems in using the artificial pancreas. We focused on the parameters, which influence the insulin infusion rate. Though several variable parameters are existing, most of them were set, based on those, proposed by Shichiri et al. Only one parameter (a-value) was left to be changed. This parameter is related to insulin infusion ratio, reacting to both proportional and diffential change of blood glucose. This value is increased when blood glucose was high. According to the a-value, three groups are classified, with small a-value(< 1. 0, n=23), moderate value (1.0≥a<2.0, n=136), and large value(≥ 2.0, n=78). [RESULTS] Destribution of the a-value ranged from 0.5 to 3.0, with most of them having more than 1.0, which was higher than that reported by Shichiri before. Therefore our ICU patients were thought to have severely deteriorated glucose tolerance. In each group of a-value, mean value of blood glucose was between 170 and 190mg/dl and thought to be good. The degree of glucose intolerance was calculated with insulin energy ratio(I/E ratio). This ratio was espeicially high in the group of large a-value.In summary, there were many patients for whom, massive amount of insulin infusion becomes necessary by using large a-value, to perform sufficient nutritional treatment.
