The Maxillary Sinus and the Orbit : -Report 2- Sinuses after the Caldwell-Luc Operation

The vertical position (depth) of anterior concave plane (ACP) of the orbial floor was evaluated by plain X-ray films of Waters' view. Evaluation was made on 23 unilateral postop. cases, 72 bilateral postop. cases and 478 cases of standard adult maxillary sinuses without surgical intervention. The postop. cases included were those in which at least two years had passed after the sinus surgery. Statistical calculations were made according to the Student-t-analysis using the average values and the standard deviation. The orbital floor (ACP) of the postop. cases showed a deeper position in comparison to the standard group and the difference was statistically significant (4.1mm in average).The lowering of the orbital floor after the Caldwell-Luc operations was caused primarily by the retraction of the scar tissue in the postop. sinuses, but small roles may also be played by intraorbital pressure and the external forces by masticatory muscles attached to the outer sinus walls. A review of the literature concerning the shrinkage of the orbit after enucleation and the subsequent compensatory development of the neighbouring paranasal sinuses was also made. Transposition of various thinner sinus walls after sinus surgery was suggested.
