耳下腺腫瘍42例の臨床統計的観察

Forty two patients with parotid tumors were treated during the last seven years in the Department of Otolaryngology, National Tohsei Hospital. There were 22 males and 20 females. The peak incidence was in the fifth decade and the average age was 47.9 years; 35 tumors were benign and 7 were malignant.About three-fourths of the benign tumors were pleomorphic adenomas. All the patients with adenolymphomas were male. Spontaneous pain, tenderness and facial palsy were observed only in malignant tumors. This finding is useful in preoperative diagnosis of malignancy. Rapid growth and large size were also suspicious factors for malignancy. For benign tumors, extracapsular removal with facial nerve preservation was performed.Two of the benign tumors recurred. One was in both lobes and the other had been operated on in another clinic. Combined treatment with surgery, irradiation and anticancer drugs was performed for malignant tumors. Three of the seven patients with malignant tumors died from local recurrence or distant metastasis. Frey's syndrome occurred in 35.3% of the patients after operation.
