Surgical results of the reconstruction of the ossicular chain with hydroxyapatite.

Ossicular reconstruction of the middle ear employing a new ossicular replacement prosthesis consisting of hydroxyapatite was performed in 67 patients with chronic otitis media with or without cholesteatoma. The prosthesis was used for total ossicular replacement (T-type), or of partial ossicular replacement (P-type). In 57 patients, the reconstructive procedure was performed in a two-stage operation. Postoperative hearing results after three months of follow up were assessed by the following criteria:1) air conduction (AC) hearing level within 40 dB (T-type: 26.1%, P-type: 65.9%), 2) AC hearing gain more than 15 dB (T-type: 26.1%, P-type: 51.2%), 3) Air-bone gap within 10 dB (T-type: 26.1%, P-type: 51.2%). One or more of the three criteria was achieved in 43.5% in the T-type and 80.5% in the P-type. Extrusion of the prosthesis occurred in only two patients and partial exposure in one.
