Short-term intensive treatment with ceftizoxime in patients with otitis media, especially otitis media with irritation of the labyrinth.

Ceftizoxime, a cephem antibiotic, was given by short-term intensive treatment to 8 patients with otitis media and 12 with otitis media accompanied by irritation of the labyrinth. Concentrations of the drug in the middle ear were determined in some patients.1) Ear discharge disappeared or decreased in 80% of the patients given ceftizoxime 1g twice a day by intravenous drip infusion for 6 days.2) Dizziness, headache and tinnitis were markedly improved by this intensive treatment. This, however, does not mean that surgery is not necessary. We think that intensive treatment with ceftizoxime is also needed in patients requiring operation.3) No noticeable side effects or abnormal laboratory findings were seen.4) The ratio of ceftizoxime concentrations in the middle ear to serum concentrations was 97.0% one hour after i. v. injection of 1g and 69.7% at 8 hours. The transfer rates of the drug were very high.5) Ceftizoxime, 1g twice a day, is expected to be sufficiently active against S. aureus, S. epidermidis and Proteus sp., considering that the drug concentrations in the middle ear are higher than the MICs for the main causative organisms and are maintained for a sufficient time.Ceftizoxime is surely effective against Pseudomonas aeruginosa infections when the frequency of injections and the dose are increased appropriately.
