Clinical and Statistical Observations of 302 Cases with Gastric Disorders by Means of Gastrocamera, with Special Reference to Chronic Gastritis

Observations were made on 302 consecutive cases at National Hospital of Iwakuni during the period of 1957 to 1962 by means of Olympus Gastro camera, Types II, III, IV and V. In this study special attentions were paid on chronic gastritis. First of all, internal surface of the stomach was gastroscopically divided into several "areas ". By investigating those areas suffering from gastritis, extension of the disease was able to be determined, Coexistence of different types of gastritis in different areas of the same stomach was termed as "complex gastritis." The results obtained from the above viewpoint were as follows: 1. Both in primary and secondary gastritis, superficial and atrophic gastritis were frequently found in the anterior wall and greater curvature and also in the anterior wall and lesser curvature above the angulus, while hypertrophic gastritis being in the greater curvature and posterior wall of the lower stomach body. 2. Primary chronic gastritis rarely localized in the areas of antrum or lower body of the stomach but usually in the areas of anterior wall and lesser curvature. Secondary superficial chronic gastritis disclosed the same tendency with predilection of the anterior wall of antrum and posterior wall of greater curvature. Although primary chronic atrophic gastritis exhibited similar findings, its frequency was less than that of superficial gastritis. Secondary atrophic gastritis showed rather marked variance. On the other hand, the extension of both primary and secondary hypertrophic gastritis was quite characteristic. Almost every hypertophic one lokalized in such areas as anterior wall, greater curvature, posterior wall and anterior wall of the angulus and the lower body stomach. In contrast, secondary hypertrophic one invaded rather higher areas as compared with primary hypertrophic gastritis, namely in the anguls, upper part of the angulus and up to the middle of the stomach body stowing non-succeeding variance in the posterior wall. 3. Complex gastritis was noted Lin 31.1 per cent of the cases with primary chronic gastritis. 10 cases of them had such type of complex gastritis as comdination of superficial and hypertrophic ones, while 3 cases being complex gastritis withhypertrophic and atrophic ones. 4. Superficial and hypertrophic gastritis sometimes disappeared during the clinical "ourse, butt atrophic one never did. The fact that superficial gastritis occasionally transformed into atrophic one in a fairly short period was thought to be due to that atrophic gastritis masked by the coexistence of superficial one became visible following the disappearance of superficial of gastritis. Although it was hard to determine the causative and accelerating factors of the atrophic gastritis with years' of history, atrohic gastritis with years' of history, atrophic gastritis itself was considered to be a final morphological stage of chronic gastritis. It would be needed to clarify the onset, various causative factors of development and prognosis of atrophic gastritis by long-term clinical observations For this purpose, gastrocamera seems to be a valuable tool with objective validity, recording and reproducibility.
