A rare congenital short femur in a 4-year-old boy was reported.At the age of two years, Z-plastic elongation of his right short femur was performed at a hospital. However pseudarthrosis occurred after the operation and shortening of the femur became so severe that he visited our hospital at the age of four years complaining of limping. Discrepancy of leg length was 13cm and mild coxa vara was accompanied, so the diagnosis of type III congenital short femur wsa made according to Hamanishi's classification.At the age of five and half years, osteosynthesis for the pseudarthrosis was performed, but the improvement of the leg length discrepancy was only 2cm.Now, 2 years after the operation, he walks quite independently using a boot with sufficiently elevated heel, freeing the hip and the knee of affected side. Leg length discrepancy has not increased in the last 2 years after the osteosynthesis.
