Elderly patients with anterior cruciate ligament deficient knees.

From May 1990 to June 1992, thirteen elderly patients over 40 years of age with anterior cruciate ligament (ACL) deficient knees underwent surgical procedures. The patient's age ranged from 40 to 62 years mean; 49 years). Seven were male and six were female. Average follow-up period was 14 months. ACL reconstruction was performed for patients complaining of instability due to ACL deficient knees. Meniscectomy was only done for patients with meniscal symptoms. Seven patients had arthroscopic ACL reconstruction using an autogenous graft (group 1). Only meniscectomy was done in 6 patients (group 2). In group 1, 5 patients were post-operatively assessed as excellent, 2 good. In group 2, 3 were graded as excellent, 2 good and 1 poor. One patient graded as poor underwent ACL reconstruction following meniscectomy. Some of the patients with radiographic osteoarthritic changes had transient hydrops and pain after surgery.ACL reconstruction was not considered as contraindicated for elderly patients solely because of age. Preoperative osteoarthritic changes influenced the postoperative results.
