• The problem of over-medication has often been called to account in Japan. Applying this to surgery, 3, 786 cases of appendectomy which were performed of a 25 year period were investigated at a certain hospital in a local city. Incidence of appendectomy among the inhabitants of that city was also studied. There has been an unusual increase in appendectomy surgery, especially in non-inflammatory cases, i.e. “catarrhal appendicitis” since about 1950. It is postulated that this increase can be attributed to the payment system of “Fee for Services” in the medical insurance system of Japan.
