Surgical liver biopsy specimens from 6 cases of primary sclerosing cholangitis (PSC) was histologically examined. Two cases showed widespread periductal fibrosis and mild lymphoplasmacytic infiltration (resembling large bile duct obstruction), 2 cases considerable loss of interlobular bile ducts, scattered epithelioid cell granulomas, nonsuppurative cholangitis (resembling primary biliary cirrhosis), and the remaining two a mixture of above mentioned two types. Portal lymphocytic infiltration was found in all cases and 4 cases showed piecemeal necrosis, and cholestatic features were found in all cases. These findings suggest that hepatic pathology of PSC is heterogeneous. Periductal lamellar fibrosis characteristic of PSC was found in all cases examined.
