To assess the immunopathogical role of immune response cells in liver deseases, the inflammatory infiltrate was examined by the method for the use of immunofluorescence on paraffin-embedded liver tlssues.<BR>Leu 2a<SUP>+</SUP>cells(suppressor/cytotoxic T cells)were significantly increased in periportal areas from patients with hepatitis. This was examplified by the increased numbers of Leu 2a<SUP>+</SUP> cells in these areas with piecemeal necrosis(PMN)from patients with chronic aggressive hepatitis.<BR>On the contrary Leu 7<SUP>+</SUP> cells(NK/K cells)in periportal areas were reduced in cases of chronic hepatitis but were increased in those of acute hepatitis.<BR>These findings indicate that suppressor/cytotoxic T cells are probable operators for chronic liver injury in the periportal liver parenchyma with PMN, and that accumulation of these cells in the periportal area may be primary mediator for the chronicity of viral hepatitis.
