Mechanisms of thrombocytopenia in acute hepatitis A.

The incidence, clinical course and possible mechanisms of thrombocytopenia were studied in patients with acute hepatitis A.<BR>Thrombocytopenia was noted during 1-2 weeks after onset, and shortened platelet life time was found. Thrombocytopenia below 15×10<SUP>4</SUP>/μl and 10×10<SUP>4</SUP>/μl was recognized in 51% and 19% of cases, respectively.<BR>The platelet counts were negatively correlated with values of GOT, GPT and positively with total cholesterol and hepaplastin test.<BR>None of patients showed the evidence of bleeding tendency clinically or DIC determined by coagulofibrinolysis test. The platelet counts were negatively correlated with splenic size. High serum levels of IgA class and IgM class circulating immune complex (CIC) were observed during the period of thrombocytopenia. Furthermore the platelet counts were negatively correlated with the levels of platelet binding IgA (PB-IgA), but not with those of platelet binding IgG (PB-IgG).<BR>These results suggested that the degree of thrombocytopenia might represent severity of acute hepatitis A, and that thrombocytopenia might be produced by the enhanced destruction of platelets non specifically bound with IgA CIC in the reticuloendothelial system.
