各種肝疾患患者血清中のType III procollagen N-peptideの動態,とくに肝癌患者における測定の意義

• 123例の各種肝疾患患者血清中のType III procollagen N-peptide (PC-III-NP)濃度をRohdeらの開発したimmunoassay法を用いて測定し次のような成績を得た(括弧内の数字は症例数を示す).正常人(15):9.90±2.60ng/ml,急性肝炎(20):14.67±6.99ng/ml,慢性肝炎(34):22.48±19.67ng/ml,肝硬変症(25):15.45±8.01ng/ml,胆石症(6):18.13±5.13ng/ml,アルコール性肝疾患(21):14.23±12.24ng/ml,原発性および転移性肝癌(17):107.11±112.90ng/ml.肝の線維化の過程でPC-III-NP濃度は肝に蓄積されたコラーゲンの量よりもコラーゲン生成の程度をよく反映するように思われた.17例の肝癌のPC-III-NP濃度は26から400ng/mlの間にあり,1例を除いてすべて30ng/ml以上であった.したがってPC-IIINP濃度が30ng/ml以上のばあいには肝癌の存在を疑うべきである.またPC-III-NPの測定は 肝癌の病勢や治療効果の判定に有用であると考えられる.
