Differences in Speech Results and of Evaluation Criteria for Cleft Palate Patients Studied in the 1975 and the 1985 Fiscal Years

• The present study was undertaken to compare the speech results of two groups of 3-year-old cleft palate patients studied in the 1975 and the 1985 fiscal years. The first group consisted of 37 patients who were evaluated during the year from April, 1975 to March, 1976. The other group consisted of 59 patients who were evaluated during the year from April, 1985 to March, 1986. The difference in the evaluation criteria for the speech performance between the two fiscal years was also surveyed. The groups studied were composed of 14 cases of bilateral cleft lip and palate, 50 cases of unilateral cleft lip and palate and 32 cases of isolated cleft palate. Palatal push back or palatal pull upward methods were used.Audio recorded speech samples of both years were evaluated and compared with each other. Evaluations of ten years before were compared with the current evolutions.The following results were obtained:(1) The pharyngeal closure indicated slightly better results for 1985, but the difference was not significant. No differences were found between the two years in the evaluation criteria.(2) No significant difference was found in the ratio of occurrence of abnormal articulation. The ratios were 67.6% for 1975 and 57.6% for 1985.(3) The distributions of each category within abnormal articulation indicated no significant difference between the two years except in that of lateral articulation. Palatalized articulation was dominant in both years; 56.0% for 1975 and 55.9% for 1985.(4) As for mild palatalized articulation, differences were found between the two years in evaluation criteria and treatment. The differences were considered to be due to an increase in the knowledge of articulation over this 10-year interval.
