In order to elucidate the pathophysiological stage of Meniere's disease, experimental endolymphatic hydrops was produced in the guinea pigs, and the correlation between the parameters of the obtained electrocochleogram and the grade of pathological endolymphatic hydrops was analysed with postoperative survival time of 3 to 5 weeks.Furthermore, thus obtained results were applied on the clinical cases to establish the usefullness of electrocochleographic technique as a tool of Meniere's diagnosis, Following items are pointed out in this study.a) By the parameter of SP/AP ratio, it is possible to detect the existence of hydrops but not grades of the endolymphatic hydrops.b) By newly defined parameter of SP area (i.e.expression of Meniere's disease broad AP area), it is able to detect not only existence of hydrops but also to predict the grades of hydrops.c) When furosemide is applied to the experimental hydrops animals, SP/AP ratio can tell the grade of hydrops at 60-75 minutes after administration.d) The mean value of 0.50 of SP/AP ratio was measured in the preoperative patient of Meniere's disease and 1.94 of SP area.After the patient underwent to endolymphatic Sac surgery, the mean value of SP/AP ratios and SP area were changed to 0.46 and 1.85 respectively.From these results, the fiducial limits of electrocochleogram's parameters between the Mfnii re's disease and normal are defined as >0.37 in SP/AP ratio, >2.75msec in width and >1.49 in the SP area.
