As a plastic procedure of the nasal cavity in atrophic rhinitis the author devised a mobilization procedure of the inferior nasal concha to the inner side by implantation of the iliac bone between the inferior nasal concha and the maxillary bone and it was published in the previous report. But the operation is difficult in the severe atrophic degenerated case or the operative failure cases of the first operation, whatever methods they were operated on. On these cases transpalatinal approach was indicated for the bone implantation in the nasal cavity, because submucous implantation of the bone in the septum and nasal floor w ill be carried out easly by this approach without failure in the posterior two thirds of the nasal cavity.At first the unilateral membrane flap is made at the inferior surface of the hard palate, having its base on the soft palate. The flap was detached from the bone and a bone flap was made at the area corresponding to the nasal floor, and the mucosa of the septum was subperiostaly detached through this wound.The iliac bone of the patient or any other material including plastic and others can be implanted, ajusting the protrusion of the septum and the nasal floor by anterior rhinoscopy.
