Exercise Test and Anoxia Test in the Diagnosis of Angina Pectoris : A Comparative Study

(1) Both exercise and anoxia tests were carried out in 41 cases with angina pectoris in order to compare their usefulness. Masters single two step test and the anoxia test following Levys method were used.(2) The incidences of positive electrocardiographic findings were 44% in the exercise and 49% in the anoxia test. Twenty-two per cent were positive in both tests. The cases positive in one of the two tests were 22% and 27%, respectively.(3) There was marked difference in incidence of anginal symptom induced by the tests: 14.8% in the exercise and 44% in the anoxia test. Anginal pain appeared in 5 of 21 cases without electrocardiographic findings at the anoxia test and only 1 of 33 cases without those at the exercise test.(4) Higher incidence of the positive exercise test was observed in angina on effort than that at rest. Anoxia test, however, had higher incidence of positivity in any type of angina pectoris than the exercise test. Angina pectoris on effort was observed in 82% in which the anoxia test alone was positive. These facts suggest that there are no essential differences between angina on effort and at rest.(5) Most of the cases with positive exercise test alone were over 60 years, while those with positive anoxia test alone and those which were negative in both tests were under 60 years.(6) Most of the cases which were previously positive in the anoxia test alone became negative at re-examination after treatment, while only one case was improved in cases with previously positive exercise test.(7) In the relationship to age and effect of treatment, cases with positive exercise test are suspected to be more serious than those with positive anoxia test.(8) Serious side effect was not observed except one case in the anoxia test.(9) One-fourth of cases with angina pectoris would be overlooked, if the anoxia test is not carried out. It is desirable, therefore, to perform the anoxia test as far as possible.
