[Abstract] We present a case in which an enlarging thin-walled cavity was observed, and surgery was performed under the diagnosis of squamous cell carcinoma. This male patient had undergone surgery for oropharyngeal cancer 7 years previously. A cyst 15 mm in size had been found in the left lung 1 year previously, and had enlarged (32 mm). The cyst lacked malignant findings, however, since it was enlarged, possibility of malignancy was considered and surgery was performed. Intraoperative pathological diagnosis was squamous cell carcinoma, but whether it was metastatic or primary could not be confirmed. Since more than 5 years had passed after oropharyngeal cancer and it was a single tumor, a lobectomy was performed under a diagnosis of primary lung cancer. Postoperative pathological diagnosis was metastatic lung cancer from oropharyngeal cancer. It is difficult to distinguish primary lung cancer with a cavity from solitary metastatic lung cancer with a cavity.
