医療的ケアが必要な特別支援学校通学児の学校生活の適応に向けた母親への看護援助モデル作成 : 母親と教員の体験の聞き取りから

本研究の目的は,医療的ケアが必要な特別支援学校通学児の適応を促すための看護援助モデルを作成することである。特別支援学校に通学する医療的ケアが必要な小学部の子ども6事例の母親と担任教員を対象とした半構成面接調査から,子どもの学校生活適応への影響要因が抽出され,「子どもの学校生活適応への影響要因の概念枠組み」を作成し,母親への看護援助が子どもの適応を促進すると考えられた。面接結果と文献検討に基づいて,「子どもの学校生活適応への影響要因の概念枠組み」と「子どもと母親のステップ到達目標とアウトカム」からなる看護援助モデルを作成した。子どもには〈学校生活への適応〉と〈生活リズム〉に関するステップがあり,母親には子どもに関する〈子どもの適応と反応のとらえ方〉,〈登校に対する価値/信念と登校の判断〉,〈子どもの健康管理〉のステップと,学校との関係に関する〈学校における子どもの体調把握/調整〉,〈学校における医療的ケア実施〉,〈教員への思い〉のステップがある。これらのステップを示したモデルは,母親が子どもの学校生活への適応を見極め子どもに適切な登校を判断することや,学校との信頼関係を作ることへの看護援助を明確にすることに有用であると考えられる。The purpose of this study was to develop a nursing intervention model for the promotion of children's adjustment to life at special schools while receiving medical care. Semi-structured interviews were conducted with the mothers and teachers of six children; factors influencing children's adjustment to school life were identified, and a framework for these factors was developed. The results suggested that nursing care targeting mothers promotes children's adjustment. A nursing intervention model was constructed from the developed framework together with a three-step goal and the outcomes of the participating children and mothers. The three-step goal and outcomes for children comprised "adjustment to school life" and "rhythm of life", whereas those for mothers comprised "child's adjustment and response", "beliefs and judgment regarding school attendance", "child's health management", "assessment of child's physical condition and health interventions at school", "medical care procedures at school", and "feelings towards teachers". The present model is expected to clarify nursing interventions allowing mothers to help their children adjust to school life by identifying proper conditions for school attendance, and to establish a relationship of trust between mothers and schools.
