[原著］亜急性の経過をとった原因不明のびまん性間質性肺炎・肺線維症の一剖検例

A case of idiopathic interstitial pneumonia was reported. A 53-year-old man was admitted to our hospital with his complaint of shortness of breath. On physical examination clubbing of the fingers was noticed and velcro rales was heard on the bilateral lower back. On laboratory data no remarkable finding was revealed. Chest roentgenogram showed diffuse reticulonodular shadow and small ring shadow of the bilateral lower lung fields. On pulmonary function test decline of VC and DLco was revealed. On blood gas analysis Po_2 was 81 torr and Pco_2 was 34 torr. Clinical diagnosis was idiopathic interstitial pneumonia. Effect of steroid was expected because of the peribronchiolar infiltration of lymphocytes and leucocytes, which was observed in the specimen obtained by transbronchial lung biopsy. After the steroid was started clinical symptoms were not only improved but rather worsened. He died after 15 months since his onset of the disease. Pathological diagnosis on autopsy was usual interstitial pneumonia. As far as indication of steroid therapy is concerned, further study regarding criteria of treament should be needed.
