[症例報告]肝細胞癌を合併したBudd-Chiari症候群の一剖検例

A 65 years old female was admitted to our hospital in October 1985 because of right hypochondralgia. Her medical history revealed that she had undergone an operation for Budd-Chiari syndrome in 1976. A diagnosis of hepatocellular carcinoma was made in April 1984. However, neither a surgical operation nor chemotherapy was carried out due to marked deterioration of liver function. At our hospital, ultrasonography showed a large tumor measuring 10.2×10.9cm in the right hepatic lobe. A marked elevation of $ \alpha $-fetoprotein (155,920 ng/ml) was also detected by serologic examination. She died of respiratory and renal failure in April 1986. Post mortal examination demonstrated the presence of membranous stenosis of the inferior vena cava and hepatocellular carcinoma based on liver cirrhosis. The long survival of this patient from the time of detection of hepatocellular carcinoma was ascribed to Grade II hepatocellular carcinoma of Edmondson's classification. The pathogenesis of hepatocellular carcinoma in Budd-Chiari syndrome was briefly discussed with relation to the present case.
