Characteristics of cytomegalovirus (CMV)-specific cell-mediated immunity in congenitally infected infants and pregnant women were studied by an in vitro lymphocyte transformation (LTF) test using the whole blood culture technique. Subjects were grouped as follows ; (1) 6 symptomatic congenital CMV-infected infants, (2) 11 asymptomatic congenital CMV-infected infants, (3) 3 postnatal CMV-infected infants, (4) 5 CMV-free infants, (5) 54 pregnant women, (6) 14 mothers of congenitally CMV-infected infants and (7) 16 non-pregnant women. 1. Results in congenital CMV infection 1) All 14 blood samples from the postnatally infected group showed higher stimulation index (SI) values than 3.0, while the CMV-free group showed below 3.0 in 36 out of 38 specimens tested. Based on these results, three-time stimulation was set as the lowest value considered positive. In the congenitally infected group, 18 out of 47 (38%) specimens, proved to be negative in terms of SI values. 2) Studies of temporal changes in CMV-LTF tests revealed that in the postnatally infected group the response turned positive in accordance with the appearance of CMV viruria and remained positive thereafter. In the congenitally infected group, the delay of positive reaction followed initial negativity, a phenomenon conspicuously observed in symptomatic subjects. 3) No significant difference was seen in phytohemagglutinin (PHA)-LTF tests among congenital, postnatal and control subjects. Thus, this disorder, which was thought to lie in specific cell-mediated immunity in congenitally CMV-infected infants, seemed to be not a defect, but a delay in lymphocyte reactivity. 2. Results in pregnant women 1) Though CMV-LTF tests were all positive in non-pregnant women, 24% of cases were negative in pregnant women. The mean SI values of CMV-LTF tests were significantly lower in pregnant women and mothers than in controls. There were no significant differences in each trimester. 2) A significant difference was seen in PHA-LTF tests among pregnant women, mothers and controls. There were several cases of negative CMV-LTF tests with a fairly decreased PHA-LTF response. On the basis of of the aforementioned observation, it seemed that CMV-specific cell-mediated immunity was lessened accompanying a decrease in non-specific cell mediated immunity in pregnancy.No affirmative data, however, were obtained as to the close relationship between CMV reactivation and decreased cell-mediated immunity in pregnant women.
