Bilateral recurrent laryngeal nerve paralysis (Midline fixation of the bilateral vocal cords) occurs on rare occasions. The spectrum of disabilities attendant to this disease range from hoarseness to complete upper airway obstruction depending upon the static position of the paralzed cords. Patients with this disease had been treated to relieve dyspnea mainly by the posterior extralaryngeal approach originally described by Woodman, after which they sometimes suffered from severe hoarseness. Newman and Work reported technical modifications of Woodman's operation which were considered of importance in enhancing operative results. Recently, by means of Newman's modifications of Woodman's operation we treated three women who complained of severe inspiratory dyspnea due to bilateral recurrent laryngeal nerve paralysis. Of the 3 cases of ages ranging from 51 to 56, 2 cases were found to have occured after thyroid surgery, while the other case was considered to have no defined cause. After operation, they were successfully decannulated and vocal quality was judged to be good. In this paper, we presented these three cases and discussed functional restoration including the modifications of Woodman's operation reported by Newman and Work.
