Recently, radiotherapy has become one of the main treatments for cancer patients in Japan. Predicting the tumor response to radiotherapy is one of the major goals of human cancer treatment. In this study, we reviewed the published reports identifying factors that influence radiosensitivity and those indicating the new targets that can enhance the effect of radiotherapy. Factors that may influence radiosensitivity can be divided into intracellular factors and extracellular factors. Intracellular factors include DNA repair, signal transduction, cancer stem cells, growth factors, apoptosis, cell cycle and cell adhesion. Extracellular factors include oxygenation and stroma. Identification of the factors that contribute to radiosensitivity or radioresistance may provide new insights into the mechanisms underlying clinical radioresistance and improve the efficacy of radiotherapy. In future, characterizations of these factors by microarray analysis could help predict responses to radiotherapy in each patient. Moreover, molecular imaging may become a noninvasive and attractive strategy to identify whether it is a radiosensitive or radioresistant tumor before starting radiotherapy.
