高齢者のライフストーリーから捉えた健康づくりの構造-独居女性高齢者の健康づくりの意味付けを通して

本研究の目的は,高齢者が語るライフストーリーにより意味付けられた健康づくりを構造的に明らかにすることである。在宅で生活を送る70歳以上の独居女性高齢者3名に対し実施したナラティブインタビューの内容から対象者個々のライフストーリーを記述し,これをもとに分析を行った。高齢者のライフストーリーから捉えた高齢者の健康づくりの構造は,生涯を通じて<健康づくりの核>を中心としながら,過去から現在,そして未来へと続く時間の流れや状況の変化,そしてからだの変化の影響を受け,<自己の身体との対話><他者との相互作用><生活行動との結びつき>が,出現・変化し,さらにこれらの健康づくりが関連しあうという構造になっていた。対象者は,ライフストーリーの中で,現在の健康づくりに対する何十年以上も前の出来事の影響を語り,自己の行く末を思い描きながら現在の健康づくりを実践していることを示した。また,一般的には健康づくりとして認知されないような内容や,本人が健康づくりとしてこれまで意識していなかった内容も,語りによって健康づくりとしての意味が与えられていた。このことから,専門職が行なう高齢者に対する健康づくり支援において,対象者自身が積極的に自己の健康づくりを意味付けていけるような関わりが重要であることが示唆された。The purpose of this paper is to investigate the meaning of health promotion structured by elderly using a life story approach. The subjects were more than 70 years old people who were living independently at home. Using a narrative interview technique, three women were asked to describe reminiscing. The data were analyzed using a qualitative methodology. The four interactive styles of health promotion were emerged from life story. <The core of health promotion> was the center throughout one's life. And <Talking with self body>, <Interaction with others><Connection with behavior>were emerged and changed under the influence of time, situation, and body condition. The participants talked about the influence of the events more than dozens of years ago on making of current health. And their healthy action was carried out while imaging their future. Narratives provided the meaning of health promotion to unconscious healthy behavior. These findings suggested that health professionals should support elderly to improve their self-meaning process of health promotion.
