がん患者と家族員の手術療法後の社会復帰における対処過程と看護援助

本研究の目的は,がん手術後の社会復帰における,患者とその家族員の対処過程の様相を帰納的に明らかにし,社会復帰に向けての看護援助を検討することである。手術を受けた成人がん患者と家族員10組の計20名を対象に,半構成的面接法と参加観察法により縦断的にデータを収集した。質的分析により,対象個々の認知した事柄と対処した事柄を抽出し,以下のことが明らかになった。手術を受けたがん患者からは,<手術しても治癒の確信が持てない><手術で失ったものに執着する>等6つの認知の主題と,《不確実性の中で耐える》《何としても自分の身体と付き合っていく努力をする》等4つの対処の主題が明らかになった。家族員からは,<がんという病気も治って欲しい><がんや手術は自分のことではない>等7つの認知の主題と,《がんは治ると信じている》《身体に関する調整や活動は本人に任せる》等5つの対処の主題が明らかになった。結果から,がんへの脅威をもちつつ,患者は人生に希望を見出そうとし,家族員は患者の生き様を見守りながら,集団としての家族を支えようとしていると捉えられる。社会復帰に向けて重要な看護援助は,患者には,がん治癒の不確さに伴う緊張状態を緩和する援助や,がん手術後の人生を前向きに生きる患者の対処行動を助ける援助等であり,家族員には,患者ケアへの熱意を支持する援助や,患者ケアへの参加の仕方を指導する援助等であると考える。The purpose of this study was to identify aspects of the process of coping with rehabilitation by patients with cancer and their family members, following operative therapy. Data was collected by means of semi-stfuctured interview in addition to longitudinal participant observation for three months. The subject cohort comprised ten patients and ten family members. Data was analyzed according to inductive procedures based on the phenomenological approach. The result were as follows: Patients recognized difficulties in their post-operative lives, as included by statements such as: "I had an operation, but I do not feel completely cured of my 'cancer'", and "I am attached to my loss by my operation". Coping strategies demonstrated by patients with difficulties in their postoperative lives were seen from statements like: "I just have to bear my uncertain situation", and "I try to feel whole and functional". Family members recognized difficulties following operations, and made statements like: "I hope that [the patient] will get rid of the cancer", and "The cancer and operation are not my business". The attempts of family members to cope with post-operative difficulties were expressed in attitudes like: "I am sure that [the patient] will get rid of the cancer", and "I leave [the patient] to control their own body and activity". These answers imply that patients struggle to find hope in their lives while still under the threat of cancer, and that family members try to support these patients as best they can. The implications for nursing care include the following: For patients, to reduce mental tension under uncertain conditions, and to assist their coping strategies; For families, to support the earnest desire to take care of their loved ones, and to guide them in the best ways to care for the patient.
