Therapeutic plasmapheresis in an acute exacerbation of the relapsing-remitting form of multiple sclerosis (MS) should be offered early for patients who did not respond to high dose intravenous corticosteroids. MS patients may not improve from exacerbation of the acute phase, despite aggressive treatment. In such cases, plasmapheresis should be performed as second-line treatment. Alternatively, it is the first treatment option in patients who cannot be given steroid pulse therapy because of complications and side effects. However, plasmapheresis is established as ineffective and should not be offered for chronic or secondary progressive forms of MS.
