慢性期脳血管疾患患者における自宅での運動実践の有用性

Background -An increasing number of studies using home- and center-based rehabilitation for stroke survivors have been conducted in Japan. However, researchers have yet to compare the effectiveness of these two types of programs. The purpose of this study was to compare the effects of home- and center-based rehabilitation programs for chronic stroke survivors. Methods -We studied two groups comprising 30 chronic stroke survivors. One group consisted of 12 survivors (HE group) who performed a home-based exercise program for 25 weeks, 4 sessions/week, and 30 minutes/session. The other group consisted of 18 survivors (CE group) who performed a center-based exercise program provided by exercise leaders at a public health center for 25 weeks, 2 sessions/week, and 70 minutes/session. Both groups received the same exercise content: warm-up, resistance exercise, gait and balance exercise, and cool-down. Measurement outcomes were assessed by 9 functional fitness test items and the Short-Form 36-Item Health Survey (SF-36) at pre- and post-interventions. Results -With regard to functional fitness, some items could improve significantly. The interaction was confirmed only in the hand-grip strength (paretic side). On the other hand, With regard to SF-36, the main effects were confirmed in vitality and the interaction was confirmed in physical functioning. Conclusions -The effects of the home- and center-based exercise programs on the functional fitness and Health-related Quality of Life were very similar. These results indicate that home-based exercise without exercise trainer is a viable alternative when chronic stroke survivors can not be provided the center-based exercise program.
