生体腎移植におけるprostaglandin E_1の効果に関する臨床的検討

The present study was designed to evaluate the effect of exogenous PGEi on the renal allografts from a living related donor. Forty-five renal transplant recipients were divided into 2 groups. Recipients in group A (n= 25) were not treated with PGEX as control group. To those in group B, PGE: (40 μg) was administered intravenously twice a day for 7 days after the operation. All recipients were immunosuppressed with methylprednisolone, azathioprine and ciclosporin. Mean creatinine clearance of the recipient in group B was significantly higher than that of group A on 3rd, 5th and 7th days after the operation (70±21, 66±13 and 73±13ml/min in group B versus 42±19, 40±13 and 46±16ml/min in group A ; p<0.05). The urinary concentration of N-acetyl β-D-glucosaminidase (NAG) and thoromboxane B_2 (TXB_2) in group B were significantly lower than those in group A. Accelerated acute rejection occurred in group A more frequently than in group B. In conclusion, these results suggest that the exogenous administration of PGE_1 improves the microcirculation and function of renal graft, by the suppression of TXB_2 synthesis.
