EVALUATION OF MAGNIFICATION ANGIOGRAPHY IN ASSESSING CARCINOMA OF THE CERVIX UTERI

In 100 patients with carcinoma of the cervix uteri, serial conventional stereoscopic pelvic angiography and serial three times magnification angiography were performed in order to evaluate their usefulness in assessing the location and the extension of the primary tumor. In addition, microangiography was also performed from 21 of 62 surgically extirpated uteri to ascertain whether there were any characteristic vasculatures present in early invasive carcinoma. It was found that a brush-like pattern tended to be observed in cases showing exophytic growth, while a wheel-like pattern tended to be observed in cases showing endophytic infiltration. Magnification angiographic stages were then compared with histopathological stages in 59 cases receiving surgical treatment to evaluate the diagnostic value of magnification angiography in the staging of cervical cancer. The results showed a sensitivity of 0.50, a specificity of 0.98 and an accuracy of 0.86 in the microinvasive carcinoma. On the other hand, in the invasive carcinoma confined to the cervix, a sensitivity of 0.96, a specificity of 0.74 and an accuracy of 0.85 were obtained. In cases with the parametrial involvement, the sensitivity was 0.90, the specificity was 0.98 and the accuracy was 0.97. This study suggests that magnification angiography could be a helpful adjuvant in the diagnosis of the tumor extent in Stage I of cervical cancer.
