"Autogenic training" as a relaxation technique is a useful health promotion technique in the modern, stressful world. The special characteristic of autogenic training, i.e. the step-by-step approach to self-control, has made it widely popular. However, "autogenic therapy" as a self-treatment for psycho-physiological disorders (psychosomatic diseases) and neuroses is not well developed. The orthodox therapeutic approach is limited to using the relaxation and anti-anxiety effects of autogenic training. It is necessary to develop autogenic therapy beyond the level of commonly used standard exercises, the first standard exercise (heaviness) and the second standard exercise (warmth). On December 1992, I received a letter that inspired me to develop correspondence autogenic therapy. The letter was from a 27-year old woman, who was suffering from a severe separation anxiety disorder based on DSM-IV diagnostic criteria. She wanted to be treated by autogenic therapy even though she could not come to my office (she had been unable to leave her home for eleven years). I accepted her request for the following reasons. 1) It was obvious from letter that she was highly motivated for treatment. 2) I understood that her ego-strength was high enough to undertake treatment by correspondence over a long period. 3) She had gained some preliminary knowledge of autogenic therapy through reading, and she was aware that she had to learn many exercises over a long time. 4) The letter was well written in terms of contents and structure, so I judged that she was intelligent enough to learn autogenic therapy exclusively by correspondence. 5) I judged that she would comply with the requirements of training because she wrote that she would be diligent in learning autogenic therapy. 6) Though she had been ill for a long time and there were difficulties in doing therapy through correspondence, she had self-efficacy and good treatment expectancy. These considerations made me decide to accept her for treatment. When I received and read the second letter from the patient, I regretted accepting her request because I felt that her biological, psychological and social conditions were too severe to be treated solely through correspondences. She had various kinds of physiological and psychological symptoms, as well as social problems among the members of her family, in addition to the separation anxiety disorder. Furthermore, she had an elder brother suffering from the same kind of her disorder for more than twenty years. In spite of these conditions, I started to try correspondence autogenic therapy with this patient.
