It has been assumed that, at the onset of exudative pleurisy that develops when the persons become allergic to tubercle bacilli, there would exist a period when the antibodies have been consumed by tubercle bacilli resulting in the state of desesitization in which the complement has been reduced in amount or is wholly absent. In an effort to determine the relationship between the various factors contributing to the mechanism of allergy, the author performed assay of the complement in blood and pleural effusion as well as the observation of the erythrocyte sedimentation rate in 20 cases of exudative pleurisy hospitalized in Ishikawa's Clinic. Investigations showed that the most marked allergic reaction was manifested in the first or the second week following the onset of the disease; during this period, a decrease or absence of the complement in blood or pleural effusion, weakening of tuberculin reaction and worsening in the erythrocyte sedimentation rate took place. As the weeks progressed, however, an increase in the complement in blood and pleural effusion, strengthening of tuberculin reaction and improvement in the erythrocyte sedimentation rate occurred.
