In this paper, author makes a factual survey about Look-out and Action to avoid collision from the points on "Time" and "Distance". Conclusions (1) Time lag from first sight of other ship to collision occured (T_1) shows very shortly, i.e., cases of less than two minutes account 41.1 percent including cases of not finding out other, (2) Distance from first sight of other ship to collision occured (L_1) is very small, i.e., cases of less than two cables account 51.8 percent including cases of not finding other, (3) T_1 is not related to ship's speed particularly, i.e., quality of Look-out is not mutually to ship's speed, (4) Time lag from time taking action to avoid collision to collision occurred (T_2) is very short, i.e., cases of less than one minute account 90 percent or more, (5) Distance from first action to avoid collision to collision occurred (L_2) shows very small, i.e., cases of less than two cables account 86.4 percent of collision but L shows larger on large ship than small ship, (6) Distance from fight of other ship to collision occurred (L_3) shows very small, i.e., cases of less than two cables account 70 percent or more. Author come to conclusion that ship's collision occurred cases being short of ample time and distance.
